ABSTRACT

As a highly prevalent global condition, myopia significantly impacts the ocular health of young individuals in China. Orthokeratology lens, as a rigid corneal contact lens, has demonstrated effective control over the progression of myopia; however, its mechanism of action remains incompletely elucidated. As one of the factors influencing visual acuity, higher-order aberrations will undergo marked changes after orthokeratology, with particular emphasis on the alterations in spherical aberrations and coma. The changes in corneal morphology induced by orthokeratology lead to significant positive increase in both spherical aberration and coma. Furthermore, the elevation of spherical aberration and coma demonstrates a negative correlation with the rate of axial length growth following orthokeratology. The interplay among spherical aberration, coma, defocus, accommodation, astigmatism, and pseudo-accommodation may constitute the underlying mechanism governing the control of myopia through orthokeratology.

ABSTRACT

AIM: To study the perception of first-order grating acuity and second-order spatial contrast sensitivity in patients with monocular anisometropia amblyopia.METHODS:A total of 715 children(715 eyes)diagnosed as monocular anisometropia amblyopia in our hospital from January 2018 to December 2022 were collected as amblyopia group, and 745 children(745 eyes)with normal corrected visual acuity were collected. The best corrected visual acuity(BCVA), first-order grating acuity and/or second-order spatial contrast sensitivity were measured, repectively. The perception ability of amblyopia patients to first-order grating acuity and second-order spatial contrast sensitivity were analyzed.RESULTS:There were significant differences between amblyopia group and normal control group in the perception of first-order grating acuity(11.58±6.10 vs. 20.27±3.47, P<0.001)and second-order spatial contrast sensitivity(0.33±0.16 vs 0.12±0.04, P<0.001). And there were significant differences between mild-to-moderate amblyopia and severe amblyopia patients in first-order grating acuity(12.10±6.23 vs. 8.13±3.70, P<0.001)and second-order spatial contrast sensitivity(0.32±0.16 vs. 0.37±0.17, P<0.05).CONCLUSION: The first-order and second-order visual pathway of the cerebral cortex in children with monocular anisometropia amblyopia have different degrees of damage. The injury of severe amblyopia is more serious than that of mild-to-moderate amblyopia.

ABSTRACT

Objetivo: Caracterizar las aberraciones corneales de bajo orden en pacientes con ametropías miópicas. Métodos: Se realizó un estudio descriptivo, observacional y transversal, con una muestra de 104 ojos de 104 pacientes adultos con ametropías miópicas y un grupo control de 104 ojos de 104 voluntarios emétropes, que asistieron a consulta de Cirugía Refractiva del Instituto Cubano de Oftalmología. Se definieron las variables demográficas, clínicas y para la cuantificación de las aberraciones corneales la elevación-depresión y el valor cuadrático medio, aportadas mediante el mapa aberrométrico del Pentacam HR. Resultados: Hubo un predominio del sexo femenino, el rango de edad estuvo comprendido entre 18 y 39 años en ambos grupos. La mediana del equivalente esférico de los pacientes de ametropías miópicas fue -3,25 dioptrías, con agudeza visual sin corrección de 0,10, esfera de -2,63 D y cilindro de -1,00 D, 91 ojos (87,50 %) tenían astigmatismo miópico compuesto. Los valores de elevación-depresión y el valor cuadrático medio de bajo orden fueron mayores en los pacientes con ametropías miópicas que los emétropes (p< 0,001). El astigmatismo vertical, desenfoque y el astigmatismo horizontal no presentaron diferencia estadísticamente significativa entre ambos grupos. Conclusiones: La miopía con o sin astigmatismo se debe estudiar y tratar como una aberración de bajo orden. Los valores de elevación-depresión y cuadrático medio son superiores en los ojos con ametropías miópicas respecto a los emétropes.


Objective: To characterize low-order corneal aberrations in patients with myopic ametropia. Methods: A descriptive, observational and cross-sectional study was carried out with a sample of 104 eyes of 104 adult patients with myopic ametropia and a control group of 104 eyes of 104 emmetropic volunteers, who attended the Refractive Surgery Clinic of the Cuban Institute of Ophthalmology. Demographic and clinical variables were defined, and for the quantification of corneal aberrations, elevation-depression and average square value, provided by means of the Pentacam HR aberrometric map. Results: There was a predominance of female sex, the age range was between 18 and 39 years in both groups. The median spherical equivalent of myopic ametropia patients was -3.25 diopters, with uncorrected visual acuity of 0.10, sphere of -2.63 D and cylinder of -1.00 D, 91 eyes (87.50%) had compound myopic astigmatism. Elevation-depression values and low-order root average square value were higher in patients with myopic ametropes than emmetropes (p< 0.001). Vertical astigmatism, defocus and horizontal astigmatism showed no statistically significant difference between the two groups. Conclusions: Myopia with or without astigmatism should be studied and treated as a low-order aberration. The elevation-depression and average square values are higher in eyes with myopic ametropia than in emmetropic eyes.

ABSTRACT

In this study, we evaluated several agronomic characteristics of S. divaricata when grown in various regions in Vietnam, including Northeast, Northwest, and Central Highlands. Among the three regions, S. divaricata grown in Northwestern Vietnam has the highest yield reaching 6.21 tons/ha, with a total active ingredient content of 0.655% (Prim-O-glucosylcimifugin 0.383% and 5-O-methylvisamminoside 0.272%). This is followed by the Central Highlands of which S. divaricata yielded 4.12 tons/ha, with a total active ingredient content is 0.543% (Prim-O-glucosylcimifugin 0.292% and 5-O-methylvisamminoside 0.251%). The lowest yield of S. divaricata was recorded in Northeast with 3.13 tons/ha, of which a total active ingredient content was 0.394% with 0.253% for Prim-O-glucosylcimifugin and 0.141% for 5-O- methylvisamminoside. With the applied analytical conditions, HPLC - DAD chromatograms are obtained with sharp peaks of prim-O-glucosylcimifugin (tR = 7.51 min) and 5-O-methylvisamminoside (tR = 20.23 min) , balanced, clear on the background of the medicinal plants Saposhnikovia divaricata (Turcz.) Schischk. In particular, the applicable analytical conditions allow for simultaneous qualitative and quantitative analysis of both prim-O-glucosylcimifugin and 5- O-methylvisaminoside. The results of building the standard curve prim-O-glucosylcimifugin Y = (16146)X – 4020.3, R2 = 0.9992, standard curve 5-O- methylvisamminoside Y = (20490)X – 6921.8, R2 = 0.9999. The quantitative results of prim-O-glucosylcimifugin and 5-O- methylvisamminoside in all regions were slightly higher than that of the pharmacopoeias of Vietnam, China and Hong Kong with the total active ingredient content not less than 0.24%. ABSTRACT

Resumen Este ensayo cuestiona a la medicina evolutiva o darwiniana por su adhesión acrítica a la teoría evolucionista para explicar las enfermedades, que deja de lado el propio proceso vital que transformó un planeta "inerte" en uno "viviente", donde el orden biológico naciente subordinó al fisicoquímico imperante para prevalecer. El orden biológico es equiparable a un "concierto armónico infinitamente diverso", que ha creado y recreado, por eones, los ambientes propicios para su propia permanencia y evolución. El arribo del homo sapiens significó el surgimiento del orden cultural que suplantó progresivamente en sus efectos, al orden biológico, al provocar cambios drásticos y vertiginosos en el ecosistema planetario que silenciaron el proceso evolutivo "sin tiempo para manifestarse". La adaptación como aptitud para sobreponerse a situaciones adversas, es un contrasentido en el "concierto armónico"; en cambio, es característica del orden cultural que impone a los humanos ambientes inhóspitos y estresantes como exigencias adaptativas ineludibles. La cualidad vital propia del orden biológico es la anticipación secuencial de las situaciones de interacción con los objetos significativos del ambiente, que posibilita la consumación de las actividades vitales básicas, emblema del estado de madurez de los seres vivos. Pensar que la evolución explica las enfermedades crónicas no solo es ilusorio, sino contraproducente, porque encubre la raíz de nuestros problemas: una humanidad en constante disarmonía entre etnocentrismos belicosos, perpetradora de la devastación planetaria, cuyo valor supremo es el lucro sin límites.


Abstract This essay questions evolutionary or Darwinian medicine for its uncritical adherence to evolutionary theory to explain diseases, which leaves aside the very vital process that transformed an "inert planet" into a "living one" where the nascent biological order subordinated the physicochemical one to prevail. The biological order is comparable to an "infinitely diverse harmonic concert", which has created and recreated, for eons, the environments conducive to its own permanence and evolution. The arrival of homo sapiens meant the cultural order emergence, which progressively supplanted, in its effects, the biological order by causing drastic and vertiginous changes in the planetary ecosystem that silenced the evolutionary process "without time to manifest". Adaptation as an ability to overcome adverse situations is a non-sense in the "harmonic concert"; instead, it is characteristic of the cultural order that imposes inhospitable and stressful environments on humans as inescapable adaptive demands. The vital quality of the biological order is the sequential anticipation of situations of interaction with significant objects in the environment, which enables the consummation of basic vital activities, emblematic of the state of maturity of living beings. To think that evolution explains chronic diseases is not only illusory but counterproductive because it covers up the root of our problems: a humanity in constant disharmony between bellicose ethnocentrisms, perpetrator of planetary devastation, whose supreme value is profit without limits.

ABSTRACT

El presente artículo se encuentra enmarcado en un periodo histórico donde se ha derrotado una vez más la posibilidad de mantener la paz. El espectro del conflicto llegó a su punto máximo una guerra fratricida que nadie esperaba y peor aun cuando amenaza con un desenlace de consecuencias catastróficas para la humanidad; por tanto nos volvemos a preguntar ¿por qué la guerra? A quien o a que atribuimos la ruptura de la paz, es difícil comprender las posiciones humanas desde el psicoanálisis, o desde la filosofía de la paz hasta la vieja concepción geopolítica de la guerra. Los Estados a pesar de entender la paz, son llevados a la defensa de sus intereses aunque signifique su propia destrucción, luego el conflicto en su ciclo de la paz a la guerra no puede ser controlado. Actualmente la guerra entre Rusia y Ucrania no sólo promueve la preocupación y la reflexión, del por qué la guerra, sino que, debe llegar a posiciones consensuadas de la comunidad internacional que perpetúe la paz imperfecta.


This article is framed in a historical period where the possibility of maintaining peace has once again been defeated. The specter of conflict reached its peak with a fratricidal war that no one expected, and even worse when it threatens a catastrophic outcome for humanity; therefore we ask ourselves again, why war? It is difficult to understand human positions from psychoanalysis to the philosophy of peace to the old geopolitical conception of war, to whom or what we attribute the breakdown of peace. States, despite understanding peace, are led to defend their interests even if it means their own destruction, then the conflict in its cycle from peace to war cannot be controlled. Currently, the war between Russia and Ukraine not only promotes concern and reflection on the reasons for war, but it must also lead to agreed positions of the international community that perpetuate imperfect peace.


Este artigo está inserido em um período histórico em que a possibilidade de manter a paz foi mais uma vez derrotada. O espectro do conflito atingiu seu auge com uma guerra fratricida que ninguém esperava, e pior ainda quando ameaça um desfecho com consequências catastróficas para a humanidade; portanto, voltamos a perguntar: por que a guerra? É difícil entender as posições humanas do psicanálise à filosofia da paz à velha concepção geopolítica da guerra, a quem ou ao que atribuímos a ruptura da paz. ABSTRACT

Abstract Four conditions of spatial contiguity of positions were used to assess sequence learning. Two sequences of 16 and 25 positions presented in two matrices of 4x4 and 5x5 respectively were used. Within each matrix, 4 (in the 4x4 matrix) or 6 positions (in the 5x5 matrix) presented spatial contiguity. The place at the sequence in which contiguous positions occurred varied across groups. In this way, spatial contiguity of the 4 or 6 positions was presented at the beginning of the sequence (Group 1), in the middle part (Group 2), at the end of the sequence (Group 3) or it was presented a sequence in which all positions occurred without spatial contiguity (Group 4). 28 undergraduate students participated. Results showed no differences among groups in the number of trials required to reproduce the sequence correctly. Number of errors was lower when contiguous positions were presented at the beginning of the sequence. These findings are explained as a possible effect of accentuation of primacy given by the occurrence of contiguous positions at the beginning of the sequence.


Resumen Cuatro condiciones de contigüidad espacial de posiciones fueron empleadas para evaluar el aprendizaje de secuencias. Se emplearon dos secuencias de 16 y 25 posiciones presentadas en dos matrices de 4x4 y 5x5, respectivamente. Dentro de cada matriz, 4 (en la matriz de 4x4) o 6 posiciones (en la matriz de 5x5) presentaron contigüidad espacial. Entre grupos, se varió el punto de la secuencia en el que se presentaron las posiciones contiguas. De este modo, la contigüidad espacial de las 4 o 6 posiciones se presentó al inicio de la secuencia (Grupo 1), en la parte media (Grupo 2), al final de la secuencia (Grupo 3), o bien, se presentó una secuencia en la que todas las posiciones ocurrieron sin contigüidad espacial (Grupo 4). Participaron 28 estudiantes de licenciatura. Los resultados no mostraron diferencias entre grupos en cuanto al número de ensayos requeridos para reproducir la secuencia correctamente. El número de errores fue menor cuando las posiciones contiguas se presentaron al inicio de la secuencia. Los hallazgos se explican a partir de un posible efecto de acentuación de la primacía, dado por la ocurrencia de posiciones contiguas al inicio de la secuencia.

ABSTRACT

Introducción: En Cuba los estándares de orden y cronología de brote dentario, usados como referencia, son foráneos. Objetivo: Describir los patrones del orden y la cronología de brote de la dentición permanente en la provincia Villa Clara, Cuba. Métodos: Se realizó un estudio epidemiológico y descriptivo de la dentición permanente en Villa Clara, entre los años 2015 y 2018. Mediante un muestreo polietápico aleatorio simple, se obtuvo una muestra de 2064 niñas y niños entre 4 y 14 años, nacidos en Villa Clara, sin alteraciones del crecimiento general y craneofacial. En ellos se observó el brote y se calcularon los promedios ponderados para las edades. Resultados: Se encontraron diferencias para las edades según Mayoral, Moyers y Águila. En más del 50 por ciento predominó la simetría y los dientes inferiores brotaron antes que sus homólogos superiores, con una frecuencia del 48,26 por ciento, los caninos inferiores brotaron después que el primer premolar inferior. Conclusiones: Predominó la simetría del brote, ocurrió por grupos dentarios separados, por períodos de reposo y un orden polimórfico. Se elaboró una tabla con las edades de brote para la población villaclareña(AU)


Introduction: In Cuba the standards of order and chronology of dental budding, used as a reference, are foreign. Objective: To describe the patterns of the order and chronology of outbreak of permanent dentition in Villa Clara province, Cuba. Methods: An epidemiological and descriptive study of permanent dentition in Villa Clara, between 2015 and 2018 was conducted. Bymeans of simple random multistage sampling, a sample of 2064 girls and boys between 4 and 14 yearsold, born in Villa Clara, without alterations in general and craniofacial growth, was obtained. In them the outbreak was observed and weight edaverages forages were calculated. Results: Differences were found forages according to Mayoral, Moyers and Aguila. Symmetry predominated in more than 50 percent and the lowertee therupted before the irupper counter parts, with a frequency of 48.26 percent, the lower canines erupted after the lower first premolar. Conclusions: Teething symmetry predominated and occurred in separate tooth groups, resting periods and in a polymorphicorder. ABSTRACT

Objective:To discuss the role of rural-order tuition-waived medical students in grass-roots public health, and put forward pertinent suggestions for further improving the effectiveness of rural directional general practitioner training programs.Methods:Through interviews with 63 graduated rural-order tuition-waived medical undergraduates from rural primary medical institutions in Sichuan Province, the standard procedures of spindle encoding and content analysis were used to analyze the influence of the training effect of rural-order tuition-waived medical students on grass-roots public health services.Results:The results showed that the training effect of rural-order tuition-waived medical students mainly reflected in theoretical learning, practical work ability, future career planning and post-graduation education, among which the training effect of theoretical learning and practical work ability was better. Rural-order tuition-waived medical students played a positive role in grass-roots public health services, mainly in the protection of key populations, home prevention and management of the elderly and chronic diseases patients, and home prevention and management of children and pregnant women.Conclusion:The measures for rural-order tuition-waived medical students to provide grass-roots public health services should include that the supporting policies of the state and schools can be continuously improved and implemented, college teachers should pay more attention to the guidance of the humanistic spirit of general medical students, and the curriculum system construction of rural-order tuition-waived medical students should highlight the courses related to grass-roots and general medicine. In the process of student training, we should construct effective practical methods and strengthen post-employment continuing education.

ABSTRACT

ObjectiveTo evaluate the quality of Mori Cortex from different producing areas by the entropy weight-technique for order preference by similarity to an ideal solution(TOPSIS), and to provide a new evaluation method for the quality control of Mori Cortex. MethodAccording to the five key indexes of color, thickness, texture, powdery and cortex remain, a subjective scoring table was designed to evaluate the appearance of Mori Cortex. High performance liquid chromatography(HPLC) was used to determine the fingerprint and the contents of multiple components(mulberroside A, chlorogenic acid, oxyresveratrol, mulberroside C, sanggenone D, sanggenone C, morusin), and chemometrics was used to explore the differential components of Mori Cortex from different habitats. On this basis, TOPSIS was used to comprehensively evaluate the quality of Mori Cortex from different habitats, and SPSS 22.0 software was used to carry out bivariate correlation analysis between thickness and appearance color with contents of seven components of Mori Cortex. ResultThose with lighter color, thicker root bark, tougher texture, sufficient powder and less cortex remain scored higher, and the top five were all from Anhui. The established fingerprint and determination methods were stable and reliable. Partial least squares-discriminant analysis(PLS-DA) screened three components with the variable importance in the projection(VIP) value>1(mulberroside A, sanggenone D, sanggenone C), which made an important contribution to the difference in the origin of Mori Cortex. Correlation analysis showed that there was a significantly positive correlation between mulberroside C with lightness value(L*) and total chromaticity value(E*ab) and mulberroside A with yellow-blue value(b*)(P<0.05, P<0.01), a significantly negative correlation between sanggenone C with b* and between morusin with L*(P<0.05, P<0.01). And there was a significantly negative correlation between mulberroside A, chlorogenic acid, and morusin with thickness(P<0.01), a clearly negative correlation between sanggenone D with thickness(P<0.05), a significantly positive correlation between sanggenone C with thickness(P<0.01). TOPSIS comprehensive scores showed that the samples from Anhui had a good score and ranked high. ConclusionThere are great differences in the quality of Mori Cortex from different habitats, and those with the close habitats show similar characteristics in appearance and component content, and lighter color and less cortex were positively correlated with the quality. Among them, the quality of Mori Cortex from Anhui is relatively good.

ABSTRACT

Implantable collamer lens(ICL)and toric ICL(TICL)are clinically common types of posterior chamber phakic intraocular lenses, they are mainly used to correct high myopia and low-to-moderate myopia of eyes whose corneal conditions are not suitable for corneal laser surgery. Accurate preoperative measurement, safe intraoperative implantation and appropriate postoperative location of lenses are critical to ensure the outcome of ICL implantable, and the appearance of V4c ICL with a central hole ensures the safety of this surgery to a greater extent. However, the intraocular position of both ICL and TICL is not invariable. Some studies have shown that the vault has a trend of decreasing year by year after ICL surgery, but its reason is unknown. And spontaneous rotation may occur after the TICL surgery, thereby causing visual loss, which is one of the main causes of TICL replacement. And postoperative changes in the position of the central hole also need attention. In addition, the visual quality of the operated eye will change after ICL/TICL implantation, but whether the change of intraocular positions of implanted lenses will affect the postoperative visual quality is also a problem that needs to be paid attention to and explored. Therefore, this paper reviewed the postoperative changes in vault, TICL axis, position of the central hole and their causes, as well as the effects of these changes on postoperative visual quality, offering valuable clinical guidance for accurate preoperative selection of ICL/TICL and surgical design, so as to improve the effectiveness and stability of ICL implantation in correction of myopic refractive error.

ABSTRACT

Objective:To investigate the effects of femtosecond laser assistcataract extraction on the levels of inflammatory factors in aqueous humor, corneal perception and high order aberration.Methods:Forty-eight patients (56 eyes) undergoing cataract surgery in the Shouguang People′s Hospital from June 2018 to February 2020 were enrolled in this study, and they were divided into observation group (24 patients and 27 eyes, femtosecond laser cataract phacoemulsification) and control group (24 patients and 29 eyes, coaxial microincision phacoemulsification) by random number table method. The levels of prostglndin E2 (PGE2), interleukin (IL)-6 and IL-1β in aqueous humor were detected before and after operation. Upper corneal and central corneal perception thresholds were recorded before surgery, 7 d after surgery, 1 month after surgery and 3 months after surgery. High order aberrations of the whole eye and cornea at 4mm pupil diameter before and after operation were recorded.Results:After operation, the levels of PGE2, IL-6 and IL-1β in aqueous humor in the observation group were lower than those in the control group: (45.62 ± 5.71) ng/L vs. (63.65 ± 5.62) ng/L, (15.25 ± 3.21) ng/L vs. (24.59 ± 6.13) ng/L, (17.16 ± 1.28) ng/L vs. (25.74 ± 4.62) ng/L, the differences were statistically significant ( P<0.05). The upper and central corneal perception threshold of observation group after operation for 7 d were higher than those in the control group: 4.55 ± 0.50 vs. 4.20 ± 0.33, 5.68 ± 0.49 vs. 5.52 ± 0.45, the differences were statistically significant ( P<0.05). The indexes of high order aberration of the whole eye decreased in the two groups ( P<0.05), while the indexes of cornea did not change significantly. The indexes of high order aberration of the whole eye and cornea between before and after surgery between the two groups had no significant differences ( P>0.05). Conclusions:Femtosecond laser assist cataractextraction can relieve inflammation, reduce the whole eye high order aberration, but not affect the cornea, and can improve corneal perception.

Objective:To investigate the association between cancer fatalism and optimistic attitudes among colorectal cancer patients, and how the above linkage is moderated by the involvement of patients′ opinions, the family members′ opinions and the physicians′ opinions in treatment decision-making.Methods:A cross-sectional survey was conducted on 64 patients diagnosed with colorectal cancer and awaiting radical colorectal cancer surgery in the Department of General Surgery of Beijing Friendship Hospital Affiliated to Capital Medical University, from January 2021 to December 2021. There were 38 males and 26 females, aged (61.2±13.0) years from 30 to 84 years. Cancer fatalism, optimism attitudes, and the involvement of patients′ opinions, the family members′ opinions and the physicians′ opinions in treatment decision-making were assessed. The patients′ cancer fatalism beliefs were measured by the " Chance" subscale of the Form C of the Multidimensional Health Locus of Control (MHLC-C), optimism attitudes were assessed by the Chinese version of the revised Life Orientation Test (CLOT-R), and the influence of patients′, family members′, and physicians′ opinions in the medical decision-making process was measured by a self-designed single factor scale. Measurement data of normal distribution were expressed as mean±standard deviation ( ± s). Measurement data of skewed distribution were expressed as M( Q1, Q3). Spearman correlation analysis and Chi-square test was used to examine the association between the main outcome variable (patients′ optimistic attitudes) and demographic and clinical characteristics. Interaction was examined by hierarchical linear regression analysis combined with simple slope tests. Results:Cancer fatalism was negatively associated with patients′ optimistic attitudes ( r=-0.35, P<0.01). Optimistic attitude of patients was significantly and negatively correlated with the influence of family members′ opinions on medical decision-making ( r=-0.25, P<0.05). There were significant positive correlations between the influence of patients′ and family members′ opinions ( r=0.50, P<0.01), family members′ and physicians′ opinions ( r=0.67, P<0.01), and physicians′ and patients′ opinions ( r=0.38, P<0.01) in medical decision making. Hierarchical linear regression analysis showed a negative association between cancer fatalism and optimism ( β=-0.32, P=0.01). This association was further moderated by the involvement of family members′ opinions ( β=-0.56, P<0.01) and the involvement of physicians′ opinions ( β=-0.36, P=0.04) in medical decision-making. Simple slope tests revealed that the negative impact of fatalistic attitudes on patients′ optimism attitudes may be potentiated when family members′ opinions have high influence on medical decision-making, while the negative impact may be buffered to some extent when physicians′ opinions have high influence on medical decision-making. Conclusions:Cancer fatalism had a negative effect on patients′ optimism. The high influence of physicians in treatment decision-making buffered the negative effect of cancer fatalism on optimism; the high influence of family members in treatment decision-making potentiated the negative effect of cancer fatalism on optimism. In the process of doctor-patient communication and shared decision-making, for patients with strong fatalistic attitudes, consideration should be given to appropriately increasing direct informational communication between physicians and patients and reducing excessive family intervention in medical decision-making, so as to enhance patients′ autonomy for treatment, promote optimism, and reduce the negative effects of cancer fatalism.

ABSTRACT

OBJECTIVES@#To detect the contents of Tangwei capsule main components with high performance liquid chromatography-quantitative analysis of multicomponents by single marker (HPLC-QAMS) method and to evaluate the quality with chemometrics and entropy weight-technique for order preference by similarity to an ideal solution (EW-TOPSIS).@*METHODS@#A symmetry C18 column and 0.1% formic acid-acetonitrile as mobile phase were used for HPLC of Tangwei capsule. The contents of 3'-hydroxypuerarin, puerarin, 3'-methoxypuerarin, methylnissolin-3-O-glucoside, calycosin, formononetin, rosmarinic acid, salvianolic acid B, dihydrotanshinone Ⅰ, cryptotanshinone, tanshinone Ⅰ, tanshinone ⅡA and cucurbitacin B in 15 batches of Tangwei capsule were determined simultaneously. The quality differences of 15 batches of samples were analyzed by chemometrics and EW-TOPSIS.@*RESULTS@#The HPLC-UV showed that 13 components had good linear relationships in corresponding concentration ranges (r≥0.9991). The relative standard deviations (RSD) of precision, repeatability and stability were all less than 2.00%. The average recovery rates were between 96.86% and 100.13%, and RSD were all less than 2.00%. Cluster analysis showed that 15 batches of samples were clustered into 3 groups. Partial least squares-discriminant analysis showed that salvianolic acid B, formononetin, puerarin, 3'-methoxypuerarin and rosmarinic acid were the main potential markers affecting the quality of Tangwei capsule. EW-TOPSIS analysis showed that the quality of S12-S15 was superior.@*CONCLUSIONS@#The analytical method established in this study can be used for the comprehensive evaluation of the quality of Tangwei capsule to provide laboratory support for its quality control and overall evaluation.

ABSTRACT

Functional membrane microdomains (FMMs) that are mainly composed of scaffold proteins and polyisoprenoids play important roles in diverse cellular physiological processes in bacteria. The aim of this study was to identify the correlation between MK-7 and FMMs and then regulate the MK-7 biosynthesis through FMMs. Firstly, the relationship between FMMs and MK-7 on the cell membrane was determined by fluorescent labeling. Secondly, we demonstrated that MK-7 is a key polyisoprenoid component of FMMs by analyzing the changes in the content of MK-7 on cell membrane and the changes in the membrane order before and after destroying the integrity of FMMs. Subsequently, the subcellular localization of some key enzymes in MK-7 synthesis was explored by visual analysis, and the intracellular free pathway enzymes Fni, IspA, HepT and YuxO were localized to FMMs through FloA to achieve the compartmentalization of MK-7 synthesis pathway. Finally, a high MK-7 production strain BS3AT was successfully obtained. The production of MK-7 reached 300.3 mg/L in shake flask and 464.2 mg/L in 3 L fermenter.

ABSTRACT

Aims: In this study, we aimed to investigate the intercorrelations between tear film break up time, measured non-invasively using non-invasive keratographic break-up time (NIKBUT), higher order aberrations (HOA) and quality of vision (QoV) in pseudophakic patients.Study Design: Cross-sectional.Methods: Thirty-five pseudophakic aged patients aged 50 years or older, and 35 control phakic patients aged 17 to 23 years with corrected visual acuity of 20/20 were included in this study. All subjects underwent similar examination including QoV questionnaire, aberrometry to measure HOA, and NIKBUT. HOA was measured with the OPD-Scan/ ARK 10000 corneal analyzer (Nidek CO. Ltd), expressed as Root Mean Square (RMS) HOA and NIKBUT was assessed using non-invasive TF-Scan module Keratograph 5M (K5M), equipped with modified tear film scanning function (Oculus, Wetzlar, Germany). Statistical analysis was performed to find the correlation between NIKBUT, HOA and QoV.Results: Patients in the pseudophakic group were significantly older (median age 66 vs. 20 years; P<0.01), had shorter NIKBUT (10.5 vs. 17.2; P<0.01), lower QoV score (1.63 vs. 0.68; P=0.04), and higher RMS HOA (0.5 vs. 0.26; P<0.01) compared to control group. NIKBUT was inversely correlated with RMS HOA (r = -0.19; p = 0.03) and RMS HOA was significantly correlated with QoV, even after adjustment for age and gender (r = -0.21; P0.04). NIKBUT <9.93s was correlated with lower QoV. The area under the curve was 0.81 (95% CI = 0.67 � 0.95, p = 0.012), and had 100% sensitivity and 61% specificity.Conclusion: Shorter NIKBUT was correlated with greater HOA and greater HOA was correlated with lower QoV. NIKBUT value of shorter than 9.93s could potentially predict pseudophakic patients who will likely experience visual symptoms leading to decreased QoV; thus, the use of artificial tears might be beneficial.

ABSTRACT

The present research focused on the green, non-toxic, low-cost synthesis of zinc oxide nanoparticles (ZnO NPs) using aqueous extract of Hibiscus tiliaceus leaves as a reducing and stabilizing agent. Thus, synthesized ZnO NPs were characterized by nanotechnological applications, i.e., ultraviolet-visible spectroscopy (UV-vis), dynamic light scattering (DLS), zeta potential, X-ray diffraction (XRD), Fourier transform infrared spectroscopy (FTIR), and high-resolution transmission electron microscopy (HR-TEM). The nanotechnological applications showed that as-synthesized ZnO NPs have bandgap energy of 2.97 eV, zeta potential of ?1.2 mV, crystalline in nature (JCPDS data card no-89-1397), and an average size of 30 to 60 nm. The FTIR showed that ZnO NPs have coated with plant secondary metabolites and assisted in the process of green synthesis. The ZnO NPs exhibited broad-spectrum antibacterial activity on Gram-positive and Gram-negative bacteria. The ZnO NPs showed potential anticancer activity against human breast cancer cells MCF-7 and determined the IC50 value as 65.83 ± 2.57 µg/mL by MTT assay. Furthermore, ZnO NPs were used as nano-catalyst for dye degradation of methylene blue, methyl orange, and malachite green with NABH4 as a reducing agent. The ZnO NPs exhibited potent dye degradation capability and followed pseudo-first order kinetics. The study concluded that ZnO NPs could be highly useful as anticancer and antibacterial agents in the biomedical field, and as an environmental cleaning agent for dye degradation in textile industries.

ABSTRACT

Purpose: Vision is one of the most important senses. The first stage of vision is the creation of the observed object抯 image on the retina. The quality of the retinal image is affected by several factors, such as diffraction, sampling on the retina, chromatic aberration, scattering and higher order aberrations. The measurement of the quality is achieved both with subjective (visual acuity, contrast sensitivity) and objective methods (PSF, MTF, Strehl ratio, RMS). The purpose of this project is the measurement of higher order aberrations of the anterior corneal surface (with a Placido corneal topographer) and of the contrast sensitivity (with a Pelli Robson optotype). Then, we will try to find if there is a correlation between them.Methods and Materials: 20 persons participated in this survey, divided in two groups of 10 persons each. The first group (group 1) included subjects up to 39 years old and the second group (group 2) from 40 years and up. The participants didn抰 have any pathological problems, except lower order refractive errors. Both eyes of each individual were included in the procedure. First, the higher order aberrations of the anterior corneal surface were measured, with the implementation of a Placido corneal topographer. This was followed by a measurement of the contrast sensitivity. The whole procedure took place under two lighting conditions, both photopic and mesopic. The examination presentation of the higher order aberrations is performed with the use of Zernike polynomials.Results: The results of the measuring procedure showed that for the first group, under photopic conditions (luminance 32.70 cd/m2) the mean value (� standard deviation) for the higher order aberrations RMS and decimal logarithm contrast sensitivity was 0.073 � 0.018?m and 1.54 � 0,16 (contrast 2.88 � 1.24%) respectively. For the second group the corresponding values were 0.080 � 0.036?m and 1.59 � 0.16 (contrast 2.57 � 1.24%). Accordingly, under mesopic conditions (luminance 1.14 cd/m2) the values for the first group were 0.252 � 0.064 ?m and 1.27 � 0.15 (contrast 5.37 � 2.06%), while for the second were 0.253 � 0.069?m and 1.32 � 0.12 (contrast 4.79 � 1.35%). Spherical aberration and coma (horizontal and vertical) were also measured for both groups under photopic and mesopic conditions. The results showed that for the first group, under photopic conditions, RMS for coma and spherical aberration is 0.033 � 0.014 ?m and 0.022 � 0.011 ?m respectively, while for the second group 0.041 � 0.027 ?m and 0.024 � 0.008 ?m. Under mesopic conditions, the relevant results are 0.139 � 0.065 ?m and 0.124 � 0.035?m for the first group and 0.149 � 0.066 ?m and 0.107 � 0.038 ?m for the second group.Finally, we should mention here that the equivalent defocus error corresponding to the higher order aberration RMS was estimated. The results for all the participants (without age separation), are 0.23 � 0.09D (photopic conditions) and 0.28 � 0.07D (mesopic conditions).Conclusions: From the statistical analysis of the results we conclude that there exists a symmetry between left and right eyes regarding higher order aberrations. Furthermore, it seems that age is not a significant factor for differences on the magnitude of higher order aberrations both under photopic and mesopic conditions as well. Similar conclusions are also reached for the contrast sensitivity measurements. In addition, we have observed that a correlation exists between the higher order aberrations of the anterior corneal surface and the contrast sensitivity, for both photopic and mesopic conditions. Correlation also exists between coma and contrast only under photopic conditions. But there is no significant correlation between spherical aberration and contrast. The age difference of these two groups has not an effect on the results. Finally, the equivalent defocus is too small, both for photopic and mesopic conditions and between each other, in order to be clinically significant.

@#AIM:To observe the therapeutic effects of standard laser in situ keratomileusis(LASIK), wavefront-guided LASIK(WF-LASIK)and small incision lenticule extraction(SMILE)on refractive errors. <p>METHODS: This study retrospectively analyzed 97 patients(194 eyes)with refractive errors admitted to the hospital between March 2020 and March 2021. They were divided into LASIK group(28 cases, 56 eyes), WF-LASIK group(32 cases, 64 eyes)and SMILE group(37 cases, 74 eyes)according to the surgical method. The uncorrected visual acuity(UCVA), diopter, high-order aberrations, tear film break-up time(BUT)and tear secretion function(SⅠt)were observed before and after operation, and related complications were recorded. <p>RESULTS: The UCVA, spherical diopter, cylindrical lens diopter and spherical equivalent were similar in the 3 groups at 1 and 3mo after operation(all P>0.05). The spherical aberrations of LASIK group, WF-LASIK group, and SMILE group at 1 and 3mo after operation decreased in order(all P<0.05). The horizontal coma and vertical coma of LASIK group and SMILE group at 1 and 3mo after operation were greater than those of WF-LASIK group(all P<0.05). The total high order aberrations of LASIK group at 1 and 3mo after operation were significantly greater than those of the other two groups(all P<0.05). BUT and SⅠt of SMILE group at 1 and 3mo after operation were significantly better than those of the other two groups(all P<0.05), without significant difference between WF-LASIK group and LASIK group(all P>0.05). The total incidence rates of complications in the 3 groups were close(P>0.05). <p>CONCLUSION: All of LASIK, WF-LASIK and SMILE can improve vision of patients with refractive errors. However, patients treated with WF-LASIK have the best visual quality after operation, and those treated with SMILE can obtained better tear film stability after operation.

@#Introduction: Laboratory tests account for 66% of clinical decision making and reducing inappropriate test utilisation is a step towards optimising patients’ care and hospital cost savings. This study aims to identify the rate and cost of redundant test requests in our centre. Methods: A cross-sectional study comprising laboratory results of 14 analytes in renal function test (RFT) and liver function test (LFT) were made. Data involved blood results from adult patients admitted to Hospital Universiti Sains Malaysia from January to December 2018. The redundant test is defined as test results consecutively normal twice and requested within 26 hours for analytes in RFT and 50 hours for analytes in LFT. Cost contributions were estimated by multiplying cost-per-test with total redundant requests. The test redundancy in different wards and disease groups were also evaluated. Results: Equal distribution of RFT and LFT requests were observed in both genders (50% respectively), with the most requests seen in the 60 – 79 years age group. More than 20% redundancy rate was observed for seven analytes (ALT, total bilirubin, sodium, urea, potassium, AST, Chloride), and overall redundancy was 19.7%, equals to Malaysian Ringgit (MYR) 669,105.00. Oncology wards and genitourinary diseases contribute to the highest redundancy rate. Conclusion: This study estimated MYR 600 thousands of saving if test redundancy were to be eliminated. The finding is hoped to serve as a platform for future intervention and policymaking. Future planning to optimise the current laboratory request system and collaboration among physicians and laboratory professionals can minimise test inappropriateness.
